GLENBROOK NORTH HOCKEY CLUB LIABILITY AND ASSUMPTION OF RISK AGREEMENT

In consideration of participating in the Glenbrook North Hockey Club (GBNHC) ice hockey program, games, practices, tournaments, and/or related events and activities: THE UNDERSIGNED ACKNOWLEDGES, UNDERSTANDS AND AGREES THAT:

1. The risk of injury from participating in the activities involved with the program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist: and, 2. I KNOWLINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, of my participation in the GBNHC, even if arising from the negligent act or omission of the RELEASEES or others, and assume full responsibility and liability for my participation in the GBNHC and for any loss, damage or injury of any nature that I may sustain arising out of my participation; These risks and dangers further include risks of sickness arising from any and all communicable disease, germs, bacteria, viruses or infections, including without limitation the virus responsible for COVID 19, whether now known or later discovered, and 3. I willingly agree to comply with the stated and customary terms and conditions for participation in the GBNHC. If, however, I observe any unusual significant hazard during my presence or participation in the GBNHC (or in the participation of the minor participant identified herein), I will remove myself (or minor participant) from participation, and shall bring such to the attention of the nearest official immediately; and 4. I, for myself and on behalf of my heirs, assigns, person representatives and next of kin, DO HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS the GBNHC and the owner of the premises used to conduct activities, and their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors and advertisers of the Event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, OR LOSS OR DAMAGE TO PERSON OR PROPERTY, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE to the fullest extent permitted by law. 

GENERAL STATEMENT

In an effort to make hockey a more desirable, enjoyable, and rewarding experience for all participants, and to further promote the organization’s objectives of providing an environment where all participants can have fun, develop skills, and learn sportsmanship, team play, and conduct themselves as responsible law abiding citizens, the GBNHC has implemented a “Zero Tolerance Policy” with respect to the behavior of players, coaches, managers, and parents who are members.

As representatives of the GBNHC, all participants have an obligation to represent themselves in an exemplary manner at all times. Participants in the GBNHC program also have a responsibility to adhere to the policies established by the GBNHC, Glenbrook North High School, School District 225, the Amateur Hockey Association of Illinois (AHAI), USA Hockey, and the Scholastic Hockey League (SHL). The GBNHC supports Glenbrook North High School’s enforcement of its academic standards and rules of behavior by taking disciplinary action when a player is determined by the school to have violated a school rule or academic standard.

By accepting the above policies, you acknowledge that you have read, understood, and agreed to abide by all Glenbrook North Hockey Club (GBNHC) rules and policies.
